Elton Gish first reported a whole one of these in his February 2001 Crown Jewels of the Wire column.  Shortly afterwards, Rick Soller reported the top part of one shown to the left here.  Rick purchased his at the Spring 1999 Wheaton, Illinois Collector's Jubilee Insulator show.  To date, these are the only ones known.  References: Crown Jewels of the Wire (February 2001)
